GUIDELINES GOVERNING CLINICAL EXPERIENCE OF STUDENT NURSES
REQUIREMENTS:
A. Accomplishment of requirements specific for each area of exposure.
B. Completion of required number of hours for each RLE area.
C. Satisfactory performance of competencies required in each RLE area.
SPECIFIC CLINICAL REQUIREMENTS:
A. UNIFORM
1. Female Students
Hospital Wards
-3 sets of uniform & apron (prescribed cut)
length of uniform should be 2 inches below the knee
apron should be 2 inches longer than the inside uniform
-3 white panty hose, 1 pair white duty shoes, caps, name plate,
watch with second hand, school ID
Community Health Nursing-Levels I to IV
-2 sets of CHN uniform (prescribed cut)
-OB bag with required contents, 1 pair black shoes
-2 brown, neutral color panty hose
2.Male Students
Hospital Wards
-3 sets of uniform (prescribed cut): white polo (Chinese collar)
and white pants
-3 pairs of white socks, white undershirts (no print allowed)
watch with second hand, pair white rubber-soled duty shoes
Community Health Nursing
-same uniform
-1 pair of black of shoes
-OB bag with required contents
B. WORKING PARAPHERNALIA
1 each of the following items:
-Stethoscope
-BP apparatus
-Black, blue & red pen
-Pencil with eraser
-Ruler 'A foot
-Bandage scissor
-Torniquets
-Ampule file
-Penlight
-Oral Thermometer
-Rectal Thermometer
-Memo note book
-Nurse's Drug Handbook
C. REQUIREMENTS IN SPECIAL AREAS
l.OR/CDR/DR
Prerequisite: Passed the OR7CDR/DR duties in Fatima Medical Center
Requirement: Report in complete duty uniform 15 mins before schedule
Bring the following:
-1 set green cap and mask
-1 white smock gown (prescribe cut)
-1 emerald green scrub suit
-5 sterile gloves for OR duty
-10 sterile gloves for DR/CDR duty
-1 pair of clean scrub shoes
-Ballpen, torniquet, bandage scissor, school ID
Orientation:
MTW group: 1st Monday of rotation
ThFS group: 1st Thursday of rotation
2. ICU/Fatima OR/DR/CDR
Requirement: Report in complete uniform 15 mins before duty
Things to bring: same as in OR Fabella
Orientation:
Sunday before the scheduled duty
NOTE: Students who did not attend the orientation will automatically receive a
grade of "5.0" and will be re-scheduled at the end of the rotation. Please
consult your clinical level coordinator.
E. RELATED LEARNING EXPERIENCE (RLE) POLICIES
All students having RLE shall adhere to the policies as stated in the
student handbook of Our Lady of Fatima University College of Nursing. In
addition to the existing policies, the following shall be observed during their
duty.
1.Clinical Assignments:
The assignments made by the Clinical Coordinator and approved by
the Dean will be strictly followed. Changes in assignments that are
deemed necessary and appropriate are made only after a consultation with
them, with written request approved by the Dean.
2.Nurse's Uniform, Cap, Hair, Make-up and Shoes:
a. Only the prescribed nurse's uniform, caps and shoes of the OLFU
College of Nursing are allowed. The length of the lady's uniform shall
be two inches below the knee.
b.The nurse's uniform is worn only in proper places or occasions, such a:
going to and from clinical duty and when attending professiona
meetings or conferences as indicated.
c.The nurse's uniform is never worn in public places such as movie
houses, public eating places, parks and in posing for advertisements.
d.Caps and aprons are worn only in the hospital. They are to be worn
throughout the clinical duty and are removed after the tour of duty.
e.Be simple and well groomed.
f.Hair style should be simple, fixed in place and short or pinned up. The
hair should not fall on the face upon stooping and should not touch the
collar. If necessary, only plain, black hair accessories are allowed.
g.Faint lipstick, powder and slight rouge and light make-up are permitted,
h. Fingernails should be cut clean and short, without any colored nail
polish,
i. Shoes should be leather-white and rubber-soled. Rubber and/or running
shoes are not allowed,
j. Jewelry worn while in nurse's uniform will be confiscated by the
Clinical Instructor and should be forwarded to the level chairman. Only
college or wedding rings are allowed.
F. TARDINESS, ABSENCES AND MAKE-UP DUTY
1.Tardiness
-A student should report 15 MINUTES BEFORE DUTY TIME
-A student is considered TARDY if he/she is not in the assigned area
during checking time or reports for duty 15 mins after the start of the
duty.
-A student who arrives late must present himself/herself to the CI so
he/she will be marked "Tardy" instead of
"Absent" Failure to do so will result in the student being marked absent.
2.Absences
-A student is considered absent if he/she
-Fails to report for clinical duty in the assigned area.
-has been tardy OVER 15 MINS, regardless of the number of duty hours
for that day.
-Absences are considered excused or unexcused.
-Excused Absences:
a.Illness with sick notice. Notification shall be made through a
telephone all and letter sent by parent with medical certificate,
certified by the school or FMC physician.
b.Death within the immediate family (nuclear) supported by a letter
from the parents and xerox copy of the death certificate.
c.Force majeure like typhoon, fire, earthquakes and other calamities
that may endanger life.
d.Official school functions.
-Unexcused Absences: absences incurred for reasons other than those
outlined for excused absences such as:
a. Attending weddings, family reunions and other family affairs
b. Extension of vacation
c.Illness without notification/medical certificate from FMC or school
physician
d.Attendance to non-school sponsored organization
e.Traffic
3.Absences in the clinical SHOULD NOT EXCEED 20% OF THE TOTAL
NUMBER OF HOURS REQUIRED FOR THAT CLINICAL
EXPERIENCES, beyond which the student will automatically be given a
grade of "5.0" whether such absences are staggered or continuous.
4.Upon return from absences, the student should fill up a make-up form (in
duplicate) and present an excuse slip with attached excuse letter from
parents/guardian or medical certificate from the school physician or FMC.
The excuse slip should be signed by the Dean or her representative for action
on the same day the student report back. Any absences not supported with
an excuse slip will be considered unexcused absence.
5.A student considered absent due to tardiness may report is she opts to
complete the requirements.
G. MAKE-UP DUTY
1.The student should make-up for excused or unexcused absences
incurred in his/her clinical practice.
2.Excused absences will mean make-up for the actual number of hours
missed.
3.Unexcused absences will mean triple make-up for the time missed in
clinical area.
4.The make-up duty schedule is prepared by the clinical Coordinator.
Prior to the make-up duty, the student should pay the make-up fee
corresponding to the number of days to be made-up. Payment should
be presented to his/her Clinical Instructor. The fee is PlOO.OO/day for
unexcused absence and P 100.00 for the whole rotation is excused.
5.After make-up duty, a make-up slip signed by the Clinical Instructor
and submitted to the Clinical Coordinator.
