Activities & Terminal Requirements for 2 Week Community Health Nursing RLE Monday-Wednesday GroupImportant Instructions: 1. Before going to your community rotation, read on COPAR and Family NCPs. These can be found in your Maglaya book. Then read and follow the instructions at the forum topic entitled
"Standard Format & Content for Community Diagnosis". Be sure to download and replicate the community diagnosis tool and bring it with you. The complete instructions can be found here:

On your first day of duty, report before 8am at the Antipolo City Health Department Main. It is a 10 minute walk from the Antipolo Church or Antipolo City Hall (Main). Almost anyone in the church or city hall knows where exactly it is.
3. Individually submit to your designated clinical instructor a 3" x 5" index card that contains your name, year section & group number, address, mobile number and a recent passport size photo of yourself.
4. Divide the following health teaching topics among your groupmates:
Influenza, H1N1, Pneumonia & TB
BCG, DPT, OPV, Measles, HepaB, Tetanus Toxoid, Nutrition
Dengue, Malaria, Leptospirosis
Hepatitis A, Cholera, Typhoid
5. You will do health teaching on the patients who visit the health centers on the days that are scheduled below. Health teachings are usually done in both English and Tagalog.
6. Afternoon sessions are dedicated to Community Diagnosis and Family NCPs.
7. During your clinical rotation, expect to have online assignments in the evening so check this forum often where these will be posted. You will send the output of your online assignments to this forum where a new thread or forum topic will be created. Typically we have a deadline for your online tasks.
After this deadline has passed, the forum closes and you can NO longer submit your online task. Not posting means you get a grade of
ZERO for this task.
8. Submission of all your terminal requirements are on the 5th duty day.
9. Present and orally defend your output on the 6th duty day.
