How To Do Your Family NCP for Thursday-Saturday Groups

IMPORTANT REMINDER TO STUDENTS: DO NOT PLAGIARIZE, SUBMIT AN OUTPUT DONE BY SOMEONE ELSE OR DO A CUT AND PASTE. REMEMBER, WE HAVE TOOLS TO DETECT COPIED MATERIAL.

ALSO SUBMIT YOUR OUTPUT ON THE 5th DAY OF YOUR ROTATION AND ORALLY PRESENT AND DEFEND YOUR OUTPUT DURING THE LAST DAY OF YOUR CHN CLINICAL ROTATION.

The output that you will submit must be both as a hard copy (paper form) and soft copy (burned on CD-R as a group). Include a CD envelope attached to the last page of your output. The book bound output may be spiral bound with a GREEN COVER and must be compiled as a group.

This will contain the following:

2. Table of Contents (Group Work)

3. Introduction (Group Work) (give the reader an introduction of the community in RICH LITERARY STYLE)

4. Acknowledgement (Group Work) (give recognition to the people, institutions, etc. that helped you create this paper in RICH LITERARY STYLE)

5. Spot Map (Group Work)
- Construct a spot map with the following:
- Directions N, W, E, S (similar to a compass)
- North should always be located at the top
- Resources in the community (brgy. Hall, school, market, church, public faucet, public toilet, etc…)
- Use color legends to indicate household interviewed, health program acceptors/beneficiaries (FIC, malnourished child, AP, PP, preschooler, TB patient, etc.)
- Give your analysis in RICH LITERARY STYLE
DO NOT use Google Earth, Google Maps or other similar sites to create your Spot Map. A sample of the Spot Map is shown below:

6. Initial Family Database (Individual)- Consult your Maglaya book on how to do this and download the guide below:

8. Family NCP (Individual) - Consult your Maglaya book on how to do this. A detailed description and a sample of a family NCP and computation can be found here:

9. Journal (Individual) - (Give a narrative day to day account of your experiences in doing your paper in RICH LITERARY STYLE)

10. List of families surveyed with their addresses (Group Work)

11. Actual survey forms (Group Work)

12. Photos (Group Work) - (Pictures of your actual community endeavor and home visits as part of your documentary proof/evidence)

Remember also that this is an INDIVIDUAL TASK but the output you will submit must be book bound as one.


STEPS:

To start doing your Family NCP, download your community diagnosis tool here:

You may need Adobe Acrobat Reader as this is in PDF format. If you do not have Adobe Acrobat Reader, download it here:
http://get.adobe.com/reader/ and install it.

Print and Replicate at least 30 copies of your community diagnosis tool. Bring this tool in your RLE rotation when you do community diagnosis and family ncp in the afternoon. You CANNOT make your output without this tool unless you prefer to create one on your own. This is the same tool you will need to make your community diagnosis.

When you go around the community, each one of you must look for 3 cases for your family NCP:
(1) Morbid
(1) Pregnant Mother
(1) Post Natal with the newborn

Identify at least 3 Problems of each of the families above. Analyze and Prioritize these problems of all the 3 cases of the families above and show your computation on all the cases you have gathered according to the following criteria:

Nature of the problem
Modifiability
Preventive potential
Salience

Use your Maglaya book as a reference.

Make your family NCP on the problem that has the highest score.

Submit with your family NCP the completed tool you used.

As this forum can be seen by anybody in the world with a digital device, there is NO EXCUSE for not knowing what your tasks and assignments are and what is going on.

Let me emphasize the importance of writing your output in proper English with a correct subject verb agreement, appropriate adjectives, adverbs, and prepositions. Please do not dare submit your work with such bad English as this:
